Fix a couple of bad pointer indirections (oops).
Lets stick to a single category of "http" for the "user_agent"

Implement php://output wrapper, which can be used to write to the output
buffer via PHPWRITE.

Tag user streams as being URLs, so that safe mode restrictions
will come into play.
