Collection fix.  getelem method now handles returning nulls
correctly and gives warning when accessing an index that does
not exist.

Make rand thread safe when ZTS is defined.

oops.  Used C++ style comments.  Make C style

Handle assign/append of NULL values to collections correctly.  Now
