Document removal of gd1 support on windows.

Drop support for php_gd.dll (gd1).
The default build (gd.dsp) is now bundled gd2.

Make apache2 handler produce php4apache2.dll which should work as a
drop in replacement for the filter. Remove max_request_per_child from
minfo() since that function is not exported by win32 version of apache2
lib.

no c++ comments..

line endings on win32 made the var_dump'ed string 4 bytes longer.  this should work on all platforms.

splitting on whitespace will be different depending on platform, so be specific with the line ending.

make test happy by adding missing functions to avoid warnings, taken from user_stream.c
