improved method for setting WITH_CGRAPH in lib/gvc/gvc.h

set WITH_CGRAPH in gvc.h

Re-add #ifdef'ed code; there is always one page even if pagination
is not supported.

Make sure all XML ids are distinct;
comment out unused page code.
