patch 8.0.0778: in a terminal the cursor may be hidden

Problem:    In a terminal the cursor may be hidden and screen updating lags
            behind. (Nazri Ramliy)
Solution:   Switch the cursor on and flush output when needed. (Ozaki Kiichi)
