merge: tidy the guide's docbook

 * Add Vim modeline
 * Reformat <screen> elements: too long or multi lines
 * Reformat <para> elements: indentation/wrapping
 * Reformat list/table elements: indentation/wrapping
 * Reformat new master commits (<para>, whitespace)
