Add a test for EVP_Digest[Sign|Verify]* with a BIO MD

If an EVP_MD_CTX is initialised with EVP_DigestSignInit_ex() or
EVP_DigestVerifyInit_ex() it should work with an MD BIO to provide
the "Updates". Test that this is the case.
